Planning for our patients during times of transitions (for example: hospital to home, home to rehabilitation facility) involves collaboration with a number of healthcare professionals. Please address the following questions:

  • How does your facility promote interprofessional collaboration during times of patient transitions?

SOLUTION

Our Facility promotes inter-professional collaboration during the patient transition in a number of ways. Successful patient transition is reliant on a number of factors. The most important one that our facility considers is inter-professional collaboration and it depends on the steps and measures that the health facility puts in place (Pinelli, Papp & Gonzalo, 2015). Therefore, my facility has practiced multiple things in making transitions successful, for instance, social processes improvement, particularly communication. Effective communication enables the smooth transfer of information from and to professionals and departments. Besides, the facility promotes inter-professional collaboration by emphasizing team structures and team attitudes that are designed in a unique way intended to foster collaboration among multiple professionals.
